About K-Brush

Four sets of K-0, K-1, K-2, K-3 were devised as a toothbrush for the treatment, but are called off in production and sale now.

Various toothbrushes are provided by many makers, but the systematization fitting a gingival state (each stage of periodontal disease) sequentially is not done these days.
It is the situation that we sort out the ones which were similar to "K brush" of Katayama from the product of each maker and use.

The "K-brush" itself of Katayama does not exist now, but the "direction for uses" includes many suggestions.

You come to be unwlling to polish if you have a pain

It is extremely important what kind of toothbrush you begin treatment with.
At the first stage, it is decided whether you can continue polishing for a long time by recognition and the sense of accomplishment of the patient such as "how much improved by this effort?"

Furthermore, it becomes the indecisive attitude that polish the point when a patient experience such as "I have a pain very and cannot polish it" when he has begun to polish.
Therefore it remains the the worst point that is the most important point without being able to polish.
Of course the brushing that polishes, and bleeds is a failure.

At the time of start of therapy, when the brushing of "the original intention" thinks that it is the origin of the long-term effort of the patient, the proper or improper of the brush of this time has a serious influence on the whereabouts of the treatment of periodontal diseases.
The brush for the treatment does not hurt at all and does not give unpleasantness.
A patient can avoid bleeding.
Nevertheless, removal of plaque is possible if you perform it carefully.
At least, a patient can keep the beautiful tooth which had plaque eliminate by the cleaning of dental clinic until next medical treatment time and do not make to pile up plaque.
Such a brush is necessary.
The point of the brushing for the treatment is "between a tooth and a tooth" and "a tooth and a gingival border".
These two small places are aims.
A patient must touch it kindly quietly to don't have a pain in both aims (do not have wounds).

When there are too many hair bunches of the toothbrush, hair bunches bump against the convex surface and are disturbed to insert in the small concave surface.
Therefore a point of a brush becomes hard to reach the outstanding dent between a tooth and the tooth to a tooth and the gingival border which became dented when you strongly lay emphasis and do not question.
Furthermore, the point of a brush that has not finished entering the part pierces weakening gingiva.
When a patient vibrate a toothbrush patiently, he would make a wound , let the gingiva bleed and produce a pain.
The "polishing that thrust a brush into the concave" for treatment is impossible with the common toothbrush.

Utilization of oil painting writing brushes and tweezers

For a long time, I processed an oil painting writing brush of the extremely soft hair into the severe place of the symptom and let the light place of the symptom use the handicraft brush which outran some hair bunches from soft hair, and adapted itself to occasional symptoms for a patient.
However, both a doctor and patient was extremely difficult together.

As for the brush for the treatment of the acute symptom, in the point of a brush that is the place to aim at effectively, the interval of the hair bunch has good wide thing with one line.
It is the factor that is important for hardness, the length of the hair.
Therefore I developed K-O of the extra-fine nylon hair bunch which they were the most effective, and neither the pain nor the bleeding gave.

A patient can polish in response to symptom improvement

It is necessary to change the way of the brushing depending on a symptom when a symptom was improved with a brush for the treatment.
When an interdental papilla part in particular became into a crater form, you insert a point of a brush between teeth properly and must completely do "Tsukkomi polishing" to stir.
K-1〜K-3 are the toothbrushes which changed length and the hardness of the point of a brush.

Attention when begin brushing

Because the patient wants to cure a bad place early, it is easy to begin brushing with the bad place.
Therefore it is easy to forget other relatively healthy parts.
At first a patient should put the bad place off and keep a relatively healthy part clean thoroughly earlier by all means in K-2, K-3, and it is necessary to reduce intraoral plaque.
Then a patient performs the brushing of the bad part last in K-O or K-1.

Long time brushing for the prevention of plaque reattachment

K-O does not aim at completely removing plaque alone.
After performing PMTC using a small tampon and the appliance at a dental clinic, and having completely kept it clean, and the greatest chief end is that by the brushing every 3-4 hours, a patient prevented the plaque reattachment namely maintain a state of no plaque, and do that he never let him had a pain.

The patient inclines the point of a brush around 45 degrees and he puts it on the cervix and polish.
Attention is necessary not to let him bleed.
Because the bleeding would damage him and causes infection, it is necessary to treat it by all means if he bleeds.
If gingiva is improved and a patient does not have a pain and does not bleed, he gradually puts a toothbrush on gingiva for a right angle and polish.
Furthermore, he turns a point of a brush toward the gums at an angle (45 degrees as standard equipment) sticking if a symptom is improved

Completely polish the interproximal space with K-1, K-2

Furthermore, it is a stage to be able to go ahead to K-1 if gingiva is improved.
As you let the point of a brush to① apply sideways a little ② apply vertically ③ apply toword the border of a tooth and a gingiva, you change the direction of the point of a brush.
If you have a pain and bleed, you return it for an original stage because you advanced early too much.

The general attention at the stage of K-1,
① The number of times may decrease three times a day, but completely removes plaque (confirm it by 2 degrees dyeing of the disclosing liquid)
② You completely do "Tsukkomi polishing" of interproximal space about a back tooth part of top and bottom, right and left.
You confirme the plaque dyeing with the oral mirror.

How to use K-2 changes equally, too.
There should be little worry of the bleeding if you continue right brushing.
You strongly gradually put a brush, and a point of a brush be careful to arrive steadily at the root of the tooth of the interdental space.

Change how to grasp brushes and polish with power

Considerable power is necessary that interdental space finally grows big than start of therapy in 3-4 weeks and lets a checkup reach it to the root of the tooth.
This solution is the change of grip ten thousand of the brush.
It is the changes from a pen grip to a method to hold with four fingers together.
In addition, an important point of the brushing shifts to training more gingival than removal of plaque from this stage.
You cannot hope for the recovery of the bone which melt and transformed if you do not get the gums with the resistance newly to be able to resist a slightly bacterial invasion.

Work hard with K-3 while being cautious of going back

A point to be careful about at time of K-3 is two.
① You did "Shake-polishing", and add the dynamic Fone's method. It is necessary to gradually take time for approximately one month.
② Once begins to be able to be dyed with a disclosing liquid properly by all means for 2-4 weeks and warns an interproximal part (interproximal) in particular and confirms that you can polish.
Because it is easy to become dull seasonally most and is easy to move back.

The above is brushing of the worst point, but there should be the point of the healthy gingiva in the oral cavity, too.
The particularly palatal part will be healthy gingiva. you brush the palatal entire surface back and forth with K-3 from a beginning to comply with a blood vessel run.
In addition, you may begin brushing with K-1, K-2 in the healthy gingiva of the interdental papilla part.
You use the four K-brushes at same time.

Medical treatment of the bleeding

You stop bleeding with saturated salt solution and Tannin liquid (boil down thin tea of the Japanese tea of ordinary quality) and sterilize it.

Art Nouveau

Katayama enjoyed everyday work namely the sense that he was surrounded in all time, and the semitransparent beauty of the human tooth, the gradation of the hue, a beautility, the molding beauty of a curve and the curved surface that continued.
Therefore, he loved the glass industrial arts such as Daume and Gallé of Art Nouveau deeply.

For flowering of the glass industrial arts of the are nouveau, the influence of Hokkai Takashima who studied in the agriculture and forestry school of the whole of Europe best is immeasurable in those days (1885-1888).
As a Japanese, Katayama felt a pride and a joy deeply in Hokkai Takashima having been concerned with Art nouveau.

Takashima was physical scientist, forest botanist, but was well-grounded in the natural observation that he wore from boyhood, and in the NANGA of Chinese painting.
At a point of the expression of his ornamental flower and creature, it was inherited with wonder by the many artists (including Daume and Gallé) of the Nancy district.

In the Katayama seminar
The photograph of the flower of the field, the glass industrial art object of Daume and Gallé came up in the interval of the slide.
It was used as a signature when a theme changed.
At the same time, it was the delicate chance that the listeners who concentrated on the lecture could draw breath.

Gaudi in me 1980

The medical practitioner of the town usually gets to know various many people by work. In dentistry in particular, there is much long acquaintance with the patient.
Work is the handwork that it is really minute, and is delicate, and the attacked organization does not recover naturally once, besides.

For the restoration, the recovery of the dynamic function as the living entity and an aesthetic sense are required.
Therefore, it is often that I sympathize than I am interested for crafts work naturally.

It is about 10 years ago that I knew an architect called Antonio Gaudi (1852-1926) and his works.
It is the beginning that the adviser of the design of my dental office reconstruction showed me "Martinell, C.: Gaudí His life, his theories, his work, Barcelona 1975".

Building of the curve constitution as it is mysterious, building which are constructed all in a curved surface except the floor, sanctuary which have been continued already building for 100 years, Sakurada Familia (Holy Family church, 1).
It was the photographs of the spires to be impossible to compare to anyone.

I knew that almost all those works stayed only in center of the Catalonia district facing the Mediterranean Sea, Barcelona which is the port town where Columbus set sail under the protection of Queen Isabella in the old days and is the town which produced Picasso, Dali, Miro.

Even so, the architect did not connect the design of my clinic with a building of Gaudi at all.
He did not seem to think about how he adopted various motifs in the photograph for the design of clinic either.

However, the things which I hoped for was shown clearly there.
These were the points what the plane structure that was always constructed in a horizontal plane and the vertical plane in recent buildings was destroyed, and were realized actually.

There was the different reason for it again.
It is the fusion of the beautiful sense with Europe and Orient.
In works of Gaudi, I watched the Orient, a beautiful sense, the recognition of the curve of the natural existence.
It is about 12 years ago.

I like music since the days of a young child.
Particularly, intense joy and sorrow are always tied to an impassioned sound.

For an intense sound of flamenco guitar and that zapateado…… Why is it?
Lamentation of the flamenco must be because it is an Oriental thing.

The movie "Barcelona story"(Carmen Amaya and Sala Reseña costarring) that I saw in about 1965 is still branded on the mind and does not disappear.

When although I started the enlargement of a building and renovation of approximately 100 tsubos ten years ago, I was troubled with various things and looked sour and moped for a design not coming to go just as me want, Leading actress Sala Reseña of "Barcelona story" came over for treatment suddenly with co-star Miguel Salvador by the performance in Japan.

I like all Spanish except bullfighting, and Spain became familiar to me through Sarah and Miguel at a sitting.
We talked feverishly about things that I wanted to hear and know in the noise and trivial routine duties under construction.
Of course, we talked about a purpose of extension and an appearance and the function after the completion of the enlargement, the dissatisfaction of a design and the enforcement again.

When they finished a performance in Japan and went back to Spain, and visited Japan again, they carried a lantern as baggage in the planes by oneself.
It is still turned on of the most expensive window.

The point which I could be never satisfied by all means, is not to have been able to break out the plane structure only with the vertical and horizontal plane

When a building is going to be almost completed, I planned Spain trip without the way of the setting of one only window which decided the appearance of this building being settled and I decided to direct it during the trip on the telephone and jumped out.

Barcelona, the excitement in the first foreign country as I showed nobody when I began to get up at daybreak and approached in front of a building of Gaudi !!
At night I strolled in the promenade to begin in the plaza of Catalonia and to lead to the shore, and then went to Tab Rao to watch the flamenco.
Although 60-year-old I was exhausted,I received emotion as became younger as for ten several years

Several years later, I visited Barcelona again and saw works of Gaudi.
In two times of trips, the hotel was Hotel Avenida Palace.
When I leave the hotel and turn at the corner, Casa Mila (2) and Casa Batllo are built in Paseo De Gracia facing a slant. Both were completed approximately 70 years ago.
This street is a representative main thoroughfare lined with the works of several best architects before Gaudi, and Holy Family church soars on foot from there to approximately 30 minutes.

The residential space of Casa Mila and Casa Batllo do not have a stiff place at all and are all "distorted", it is comprised of a curve and a spiral.
Polychromy (many colors decoration) that the color was also studded with Arab tiles. Materials of the door and the room decoration are forging steel, a sandstone, a beach rock, marble, onyx concrete and ceramics.
The building part and the furniture which touch a hand, and adjust to skin are made as the will and hand of craftsmen cooperating with Gaudi are unified by one, it is still there.
In power and movement and balance over baroque, are nouveau, there are all meanings and stillness and ease.

And the building of the Holy Family church which will be continued for 100 years is still pushed forward calmly from now on if for expiation of the modern machine civilization.

I intend to travel in the rest to Cripta de la Colonia Guell (chapel 1908-1916 for worker colonies, 3 or 4) which there is to product of the most later years of Gaudi, suburbs approximately 20 kilos of Barcelona.

Definition of the health of WHO

Health is a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity.

Primum non nocere

Primum non nocere is a Latin phrase that means "first, do no harm."
The phrase is sometimes recorded as primum nil nocere.

Non-maleficence, which is derived from the maxim, is one of the principal precepts of bioethics that all healthcare students are taught in school and is a fundamental principle throughout the world. Another way to state it is that, "given an existing problem, it may be better not to do something, or even to do nothing, than to risk causing more harm than good." It reminds the health care provider that they must consider the possible harm that any intervention might do. It is invoked when debating the use of an intervention that carries an obvious risk of harm but a less certain chance of benefit.

From Proverbs Chapter 1

Ars longa, vita brevis, occasio praeceps, experimentum periculosum, iudicium difficile. Nec vero solum seipsum praestare oportet oportuna facientem: sed et assidentes, et exteriora.

Life is short, and Art long; the crisis fleeting; experience perilous, and decision difficult. The physician must not only be prepared to do what is right himself, but also to make the patient, the attendants, and externals cooperate.

Hippocratic Oath

サンプル >> read Original version in Greek

I swear to fulfill, to the best of my ability and judgment, this covenant:

I will respect the hard-won scientific gains of those physicians in whose steps I walk, and gladly share such knowledge as is mine with those who are to follow.

I will apply, for the benefit of the sick, all measures which are required, avoiding those twin traps of overtreatment and therapeutic nihilism.

I will remember that there is art to medicine as well as science, and that warmth, sympathy, and understanding may outweigh the surgeon's knife or the chemist's drug.

I will not be ashamed to say "I know not," nor will I fail to call in my colleagues when the skills of another are needed for a patient's recovery.

I will respect the privacy of my patients, for their problems are not disclosed to me that the world may know. Most especially must I tread with care in matters of life and death. If it is given me to save a life, all thanks. But it may also be within my power to take a life; this awesome responsibility must be faced with great humbleness and awareness of my own frailty. Above all, I must not play at God.

I will remember that I do not treat a fever chart, a cancerous growth, but a sick human being, whose illness may affect the person's family and economic stability. My responsibility includes these related problems, if I am to care adequately for the sick.

I will prevent disease whenever I can, for prevention is preferable to cure.

I will remember that I remain a member of society, with special obligations to all my fellow human beings, those sound of mind and body as well as the infirm.

If I do not violate this oath, may I enjoy life and art, respected while I live and remembered with affection thereafter. May I always act so as to preserve the finest traditions of my calling and may I long experience the joy of healing those who seek my help.

Written in 1964 by Louis Lasagna, Academic Dean of the School of Medicine at Tufts University, and used in many medical schools today.

Not the problem only for teeth, it is the wish of the dentist to have you live as a human being with health of the heart and the body, and spend the valuable splendid life.
(The quotation from Ehime dental association "dental prevention handbook")

Edward Livingston Trudeau

Guerir quélquefois Soulager souvent Consoler toujours

Edward Livingston Trudeau (5 October 1848 - 15 November 1915) was an American physician who established the Adirondack Cottage Sanitarium at Saranac Lake for treatment of tuberculosis. Dr. Trudeau also established the Saranac Laboratory for the Study of Tuberculosis, the first laboratory in the United States dedicated to the study of tuberculosis. He was a public health pioneer who helped to establish principles for disease prevention and control.

Ambroise Paré

Je le pansai, Dieu le guérit I bandaged him and God healed him

Ambroise Paré was an innovative French surgeon who served as royal surgeon for a number of French kings, including Henri II. Having been apprenticed to a barber, Paré joined the army in 1536, and spent much of the next 30 years as a military surgeon. He improved or invented many techniques, especially in the treatment of war wounds.

It was common practice at the time for surgeons to seal wounds by the use of cauterisation but, in his first job as a war surgeon in 1536, Paré ran out of the boiling oil which was used to seal the soldier's wounds in this way. Instead, he made a tincture of egg yolk, turpentine and oil of roses. The following morning, and to his amazement, the soldiers who had been treated with the tincture were in a much better condition than those who had been treated with boiling oil.
Paré also rejected cautery to seal wounds after amputation. Instead, he used ligatures to tie off the blood vessels.

Awareness, Self-help, Independence

A person recognizes the thing which is supposedly the hidden etiology in one's lifestyle to cure a disease. (awareness)
A person knows the real nature of illness, and it is necessary to make measures to remove an etiology properly, and to carry it out. (self-help)
A person must repair the life that became an illness by oneself. (independence)

This Awareness, Self-help, and Independence are very important.

Means to save without extracting a tooth

Those discussions which "You extract a tooth, or do not" and "When do you extract a tooth ?" are unnecessary and sterile.
"What would you do to save a tooth without extracting it ?" is important for a dentist.

The tooth is a window of the health

We regarded it as the problem only for teeth, and the tooth does not improve.
The tooth is a window of the health and is a mark and is an entrance.
It is previous notice of the health at the same time, and this is because it is a total financial statement again.
The healthy acquisition of oral functions is the starting point of much healthy exercise, and a goal of the healthy maintenance again.

For distinction and the use of the effective, appropriate method in the motivation, the will , the effort and the deep insight for the action of the partner of the leader are necessary
This attitude is necessary in the school education in the same way

What is Youth?

This is a story to come out in the lecture of the Katayama seminar, but is a sentence of Samuel Ullman about "youth and age".

How he became young at sixty?

Horace Fletcher:1849~1919

Horace Fletcher that was a rich clock dealer had become the body of the semi-invalid because of bad the life of the dainty food enthusiast, but regained the physical strength that was youthful by biting food as one of the hygiene well thoroughly.
Besides, a physiologist, a nutritionist of the U.K. and the American Oxbridge changed the heart of the kinesiologist, and this came to get a global evaluation in those days. Fletcher wrote a book from this one's experience, and the Fletcher ism spread out in the world.
It was translated into Japanese with the title of a book called "The complete chewing method", and published in 1940 by association of Japan life.

Fletcher ordered ingredients with youth of approximately 40 years old from all over the world and employed five cooks for exclusive use of oneself and enjoyed dainty food in those days. As a result, I caused abnormality to heart, and the digestive organ system became just before a disease, too. 171cm, the weight were nearly 100 kg at 40 years old. I thought, "I am anxious about an end with such a body" and was going to take out life insurance, but I failed it by the result of the physical checkup, and a contract of the life insurance has been declined.
Fletcher that was shocked by this consulted a skilled physician over the U.K. and learned from a famous nutritionist, but was not able to get a result to understand. I asked various hygiene by oneself and I really tried it, but was not able to readily meet any thing.
In such an occasion, Fletcher hears the story of the British prime minister "W. Gladstone" accidentally.

U.K. for four quarters received such a question from one newspaper reporter at the Prime Minister's office.
In spite of "85 years old, why are you well?"
In contrast, Gladstone answered in this way.
Because "the sky gave 32 teeth to us, I always chew 32 times.
Persuade children in this and let you protect it. "
Because "I chew enough, and there are our 32 teeth, I bite food 32 times.
It is my hygiene to perform this for life.

Therefore the weight decreased in a moment when Fletcher tried the same thing, and it practiced that "it well bit it, and it ate "it was simple, and it cooked a fresh thing "to really eat of the sense of hunger only when it sprang out", and to eat" while tasting it slowly", and physical strength recovered. There was little quantity of the meal and finished it when I ate while changing it into a very simple meal judging from the times of the former dainty food such as the fresh vegetables and fruit, fish or egg, and tasting a meal slowly as if well biting it thoroughly. I had you check this result in the nutritional classroom of the university and proved how "hygiene to chew" was superior.
